PR Release Advice

I am trying to develop a couple of communications. 1) a pr release and 2) requests for giveaways. I am at a loss as to how much to put it in.

Anyone have a good template or advice on what to include?

Thanks for the advice. Very helpful. Its taking me awhile to get this going but I am determined to do it. I am a former marketer so I know that brevity is the key especially in this market.

I used wufoo to create a review/ giveaway request form. It's really simple.
