Personally I use Twitterfeed to post my giveaways to both my Twitter (@DealiciousMom) and my Facebook Fan page. This way I do not have to put any extra effort to post all my giveaways and other posts in all these different places.
As far as getting followers there are lots of ways. Find peoples blogs you like and add them. Then look at who they are following on Twitter and add the people you recognize or like. People will start to follow you back especially if you offer fun or interesting Tweets. There is also Mr Tweet - this site offers you recommendations on who is worth following.

To use google forms (documents) you will need a google account and then you can go here to sign in and set up a new form. After you sign in you will want to set up a new form and take it from there. Let me know if you have any new questions - feel free to email me.

Not sure about Google Forms and blogspot but I assume you are fine sense google forms just generates a code that you place in your post. You can still have extra entries for leaving comments and etc.

That reminds me ALOT of review bloggers have an extra entry for following on Twitter and retweeting about the giveaways so that is another good reason to have a Twitter account. Companies like to see their products "get tweeted" about.
